Passports
NOTE: Due to a software upgrade, the Prothonotary's Office has temporarily suspended passport operations.
The Dauphin County Prothonotary's Office is an authorized passport acceptance facility. You may bring a completed form and required documentation to the Prothonotary's Office by appointment ONLY Monday through Friday. Please call our office to set up your appointment.
What you will need:
Form -
- The Prothonotary's Office has blank forms available to the public;
- Additionally, forms are available online: travel.state.gov for DS-11 form
Proof of U.S. Citizenship -
- State Certified Birth Certificate - Long Form Only;
- Both parents' names must be included on the birth certificate;
- To obtain a birth certificate go to PA Vital Records or your Out of State Vital Records;
- Naturalization Papers - Original only;
- Expired U.S. Passport.
Proof of Identity -
- Valid Driver's License or State-issued Identification Card
- If an applicant presents an out-of-state driver's license, additional identification is required.
- Health insurance card;
- Work ID;
- Social security card;
- Military ID.
- School ID
- If an applicant presents an out-of-state driver's license, additional identification is required.
Photographs -
- 1 color passport photo (2"x2") taken within the last 6 months.
- Passport photos can be taken in the Prothonotary's Office for a $10 fee.
Fees (for each application) -
- Passport Books: Check or Money Order made payable to the U.S. Department of State;
- $130.00 if applicant is 16 years of age or older;
- $100.00 if applicant is 15 years of age or younger;
- Passport Card: Check or Money Order made payable to the US Department of ;
- $30.00 if applicant is 16 years of age or older;
- $15.00 if applicant is 15 years of age or younger
- Expedited Processing: An additional $60 added onto the payment to the US Department of State;
- For each application: Check, Money Order or Cash made payable to the Dauphin County Prothonotary in the amount of $35.00;
- For pictures taken by the Prothonotary's Office: Check, Money Order or Cash made payable to the Dauphin County Prothonotary in the amount of $10.00
- Can be combined onto one check included with the application fee.
Additional Information –
- Original citizenship documentation (i.e.: Birth Certificates, Naturalization Certificate, Expired Passport) must be submitted along with your passport application. These documents will be returned to the applicant in a separate envelop sent via US first class mail.
- While we do not accept adult Passport Renewals, we do have the forms available for public. Applicants can also visit travel.state.gov and look for the DS-82 form for a Renewal. Adult renewal forms are mailed directly to the passport agency by the applying party.
- It currently takes eight to eleven weeks for routine passport processing. Expedited services currently takes five to seven weeks for processing and an additional fee is required.
